Notice to clients and prospective clients:
Under the law, health care providers need to give clients who don’t have insurance or who are not using insurance an estimate of the expected charges for medical services, including psychotherapy services.
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare services, including psychotherapy services.
You can ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule a service, or at any time during treatment.
If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.

Can you complete a Family Medical Leave Act (FMLA) form for me to give to my employer? Can you complete an American’s With Disabilities Act (ADA) form for me to give to my employer? Can you document for my attorney what you believe my work limitations are? Can you write a note for me to get accommodations based on my disability? Can you write the retirement board recommending that I retire due to a psychological injury?
No, the Arizona Board of Behavioral Health Examiners has determined that it is outside the scope of a Licensed Professional Counselor or a Licensed Independent Substance Abuse Counselor to document any evaluation of a client’s disabilities, required accommodations, work limitations, or capacity to work. I recommend you speak to your primary care physician or other specialist to complete the documentation you require.
